AFTER-SCHOOL SOCCER REGISTRATION & CONSENT FORM (FALL 2026) Location: Hebrew Language Academy (HLA), 2186 Mill Ave, Brooklyn, NY 11234 Schedule: Tuesdays & Thursdays | 3:30 PM – 5:15 PM Program Duration: October 1, 2026 – December 23, 2026 Tuition: $600 per student / semester (Sibling Discount: -$100 off per registered sibling)

Welcome to our after-school program for students from Pre-K to 6th grade! Schedule: Wednesday: 3:30 – 5:00 PM Friday: 1:30 – 3:00 PM Late Pickup (Friday): Available at 3:30 PM for an extra $5. Plans: Membership Price Notes 2x per week $220 / month Or $800 for the full winter season 1x per week $150 / month Fixed day (Wednesday or Friday) Single class $40 Subject to availability Payment Terms: Payments are due within the first week of every month. Post-payment is not available. Payment Methods: Cash, Zelle, Venmo, Credit Card (5% processing fee applies). Program & Activities Our unique methodology combines intensive soccer training with learning sports vocabulary in Hebrew. Students learn commands, on-field communication, and team cheers. In addition to soccer, the program includes: Gaga ball and Dodgeball to develop reflexes. Athletic challenges to improve overall physical conditioning. Training is led by 3-4 qualified coaches. Location: Practice takes place outdoors (school backyard) when weather permits; indoors when it is cold or rainy. Uniform A professional look is part of our athletic discipline. Kit: Adidas Squadra 26 Grey (jersey, shorts, socks). Cost: $72 per set. Ordering: After registration, you will receive a personalized link from our operational manager, Amaliia, via email. Note: If you already have the uniform from the previous season, you do not need to purchase a new one. Mandatory Tournaments (OLAM Soccer Tournament) Participation in these tournaments is a required part of the curriculum for all students: Fall Classic: Oct 4, 2026 — Outdoor at the Metro Brooklyn Soccer Club field. Winter Cup: Jan 24, 2027 — Indoor at HLA (after MLK weekend). Olam Cup: May 9, 2027 — Outdoor at HLA (closest Sunday to Yom Ha'atzmaut). Includes entertainment for the family, food, medals, and trophies. Rules & Requirements For the safety and efficiency of our training, please follow these rules: Dress Code: Jeans are strictly prohibited. Non-sport footwear (flats, flip-flops, heels, etc.) is forbidden. Being on the field barefoot or in socks only is not allowed. Footwear: Because indoor training takes place on parquet flooring, cleats for natural grass are prohibited. Please wear indoor soccer shoes or sneakers. Gadgets: The use of phones and other gadgets is prohibited. Exceptions are made only for emergency calls to parents. Schedule Changes: Start times may change on early dismissal days. Monitor updates in the 360 app and the WhatsApp chat. Operating Policy: We train during school holidays (except for the winter break before New Year's), unless the school is undergoing deep cleaning or construction. Makeup Policy Missed classes due to personal reasons (family trips, etc.) will not be made up or compensated. Makeups are only provided for absences exceeding 6 classes due to documented illness or injury. Welcome — Let’s Get to Know Each Other If you’re here, it means you’d like to see how your child feels on the field with us before making a long-term decision. That’s exactly what this session is for. Our Free Tryout Session gives your child the opportunity to: • train with our coaching staff • experience our training environment • feel comfortable and confident on the field • be observed for appropriate group placement This session is not a test and not a competition. There are no expectations and no pressure — just an opportunity for your child to play, learn, and enjoy the experience. At the same time, it allows our coaches to: • understand your child’s age, level, and readiness • recommend the most suitable program (Recreational or Development) • ensure proper group placement for safety and progress You already know our approach. This session simply helps us make sure the fit is right — for your child and for our program. Please complete the registration form below. The first league games will begin on September 20. The league fee is $350 per player for one season. Payment can be made by Cash, Zelle, or Venmo to phone number 9293778886 . Available Travel Teams: 2012/2013 2013/2014 2014/2015 2016/2017 2017/2018 2018/2019 League Fee Deadline: !All league fees must be paid no later than the first week of September! Uniform Requirements: Players who already have their game uniforms from the previous season do NOT need to order new uniforms. All NEW players MUST order the required game uniform sets, including a black and white jersey with their assigned number. Players will not be able to participate in league games without the required uniforms. Travel Team Player Requirements: -Players are expected to attend all practices regularly. -Players must give 100% effort and commitment during practices and games. -League games cannot be missed without a valid reason. -Players must always bring both game uniform sets to every game. -Players are expected to listen to and follow all instructions from their coaches. !By registering for a Travel Team, families acknowledge and agree to these requirements and understand the level of commitment expected throughout the season!
