AFC Wolves Registration form This form is to be completed by All Parents who wish for their child to be registered to AFC Wolves for the 2024/25 season in the MJPL with matches played on Saturdays. For more information on registration please refer to your AFC Wolves Welcome Pack. MJPL memberships are £45 per month.

AFC Wolves Registration form for Development centre sessions This form is to be completed by All Parents who wish for their child to be registered to AFC Wolves Development Centre. Sessions on Wednesdays 6pm-7pm City of Wolverhampton College, Wellington Road, WV14 6BT. £25 per month membership.
